Please submit the completed "Celebration of Life" form in PDF or Word, along with a headshot or high-resolution photo of the departed Rotarian or spouse, to me, Sonya Mendez via email at sonyamendezrotary808@gmail.com by Friday, April 12, 2024.

Celebration of Life Form Instructions:

1. E-mail the Form and Photo: Send the completed form and a JPEG photo of the Rotarian or spouse who passed away between May 31, 2023, and April 12, 2024, to sonyamendezrotary808@gmail.com.

2. Include Relevant Details: Provide the full name, nickname, date of birth, date of death, and Rotary Club affiliation of the departed individual.

3. Share Their Story: Write a brief paragraph highlighting their Rotary achievements, personal history, and any other noteworthy information. Include the number of years they were a Rotarian and/or a member of your club.

4. Information Submission: Include your name, phone number, and email as the person submitting the information.

James “Kimo” Smith

Known to his close friends as “Kimo,” James passed away on July 7, 2023. Kimo loved anything to do with the ocean - diving, spearfishing, boating, and even beach cleanups. Kimo was a past president of the Rotary Club of _________ and a member for 25 years. He is survived by his wife Mary and sons James, Jr. and George.
